Wisconsin Immunization Program - Public Immunization Record Access

Parents and Guardians – It’s that time of year again!  School starts in about a month and half – or less, and one piece of information your students will need in elementary school, high school, and college or university is their immunization records.  For many Wisconsinites, you can go online to get a copy of your student’s  record.   The website is the Wisconsin Immunization Program – Public Immunization Record Access, and just click on that link to go straight over there.  When you get to the website, select a link under Public Immunization Record Access, and a screen will appear asking for first name, last name, birth date, and social security number or Medicaid ID or provider chart number.  If your student received their immunizations in another state, there is the possibility that they may not be entered into the record.
